Governed AI is a production discipline — not a product, not a feature, not a marketing label. It means every AI agent in your system has a proof obligation, an independent verifier, and an operating procedure that together ensure the system can't silently fail.
The enterprise AI failure rate sits at 70–85%. Not because the models aren't good enough — they are. The failures come from missing operating discipline: no verification, no gates, no measurement, no accountability. Governed AI is the fix. Without governance, AI agents declare themselves "done" without independent verification. They hallucinate facts that nobody checks. Governed AI doesn't mean slow AI. The system is fully autonomous for low-risk operations — data processing, formatting, internal calculations. But external sends, payments, destructive actions, and anything that touches a customer or compliance surface stops for human approval. The governance is proportional to the risk.
